Additional information and forms are required. Chidlren will not be allowed to attend until these forms

are turned into the office. It often takes a while to gather this information, so please allow yourself the necessary time.

We offer many ways for parents to enrich their children’s experience at Kiddie Kingdom. From volunteer opportunities to instructional workshops, we encourage parents to play an active role in their child’s development.

ORIGINAL BIRTH CERTIFICATE This will be returned to you.
STATE OF VA PHYSICAL EXAMINATION & IMMUNIZATION FORM Your child’s immunization must be up to date and a recent physical exam is required.
PARENTS AS PARTNERS Required for all preschool age children.
CHILD AND ADULT CARE FOOD PROGRAM ENROLLMENT FORM This form verifies your child’s enrollment at Kiddie Kingdom to the USDA food program in which we participate.
ADDITIONAL ENROLLMENT FORMS Photography & Antibiotic Permission, Field Trip Emergency Evacuation Forms

Children are required to have updated information on immunizations once between the child’s fourth and sixth birthdays. This must be done prior to entrance into Kindergarten. Please note that, all children born on and after January 1, 1997 will be required to document having received at least one dose of varicella (chickenpox) vaccine. Documentation should be included in the immunization section of the State of Virginia Physical Examination and Immunization Form.

It is expected that children enrolled will remain so on a continuous basis, however, in order to sustain enrollment in all of our classes, parents of currently enrolled students complete Intent Forms for continued enrollment for the upcoming summer and following school year. Intent forms will be issued, and your child’s space will be guaranteed upon receipt of this intent form and the payment of the re-enrollment fee. You will have one month from date of issue to return the intent form. At that time, registration will be opened to the general public and applications will be taken on a first come, first served basis.
